血管性血友病因子(von Willebrand Factor, vWF),是由第12號(hào)染色體的短臂所編碼的糖蛋白。vWF可同時(shí)與膠原纖維和血小板結(jié)合，當(dāng)血管破裂時(shí)大量血小板以vWF為中介，粘附在膠原纖維上，形成血栓，得以止血。在正常的生理?xiàng)l件下,絕大部份由內(nèi)皮細(xì)胞合成,其余則由巨噬細(xì)胞生成。前者產(chǎn)生后多儲(chǔ)存在W-P小體（Weibel-Palade body，WPB）內(nèi), 后者則儲(chǔ)存在血小板的顆粒中。它與一系列心血管疾病如動(dòng)脈粥樣硬化、急性冠狀動(dòng)脈綜合征、心房顫動(dòng)等均關(guān)系密切，vWF是一種重要的血漿成分。vWF在止血過(guò)程中主要由兩種作用：①與血小板膜GPⅠb-Ⅸ復(fù)合物及內(nèi)皮下膠原結(jié)合，介導(dǎo)血小板在血管損傷部位的黏附；②與因子Ⅷ結(jié)合，作為載體具有穩(wěn)定因子Ⅷ的作用。此外，vWF也能結(jié)合GPⅡb-Ⅲa，參與血小板的聚集過(guò)程。正常人血漿vWF濃度約為10mg/L。在內(nèi)皮細(xì)胞受刺激或損傷以及集體處于應(yīng)激狀態(tài)時(shí)，血漿vWF水平升高。vWF及某些止血與纖溶成分都是血栓性疾病的獨(dú)立危險(xiǎn)因素。相反，當(dāng)vWF基因發(fā)生缺失、插入點(diǎn)突變、剪切點(diǎn)替換或提前形成轉(zhuǎn)錄終止信號(hào)時(shí)，導(dǎo)致血漿vWF量有顯著減低或有質(zhì)的缺陷，不能完成其正常的止血功能時(shí)，即為vWD。
